Birth is the part of the novel "The Citadel."
Andrew Manson had just completed with his medical studies who was now assisting Dr Edward Page in a small mining town Blaenelly.
.....here it begins: Andrew was coming back disappointed after meeting his girlfriend Christine. As he reached his home, he saw Joe Morgan who was waiting for the young Doctor restlessly. His wife was in a poor condition as she was giving birth to their first child, after twenty years of their marriage.
They walked down the street, it was midnight. As they reached door number 12, Morgan's house no. the doctor entered inside while Joe waited outside, stressed.
Andrew saw the patient lying , beside her was her old mother, in her seventies. He found out that the baby can come out anytime and had to wait in their house as the patient was a middle aged woman. It was half past three, the nurse in the room screamed to the doctor. It was time for some work, as the doctor was operating the mother and after an hour a baby boy was born, lifeless. The patient lay as she was given anaesthesia and the old woman looking at the stillborn baby. Andrew was competely in shock, and tried all possible efforts to get back the child to life. The baby's body was white indicating towards asphyxia, pallida. He had seen a similar condition in Samaritan, studying medical. He quickly asked the nurse for hot water, cold water and basins. Baby laid once in hot water then in cold, nothing happened. Andrew sat there helpless, remembering how much the old lady wished for a grandchild.
He again tried, for the last time, rubbed his back, started breathing in the child's mouth. And then it was a miracle, the heart started beating as felt by Andrew, there was a sense of life lying in the hands of Andrew Manson who grew joyful, had a feeling of achieving success and having doing something real.
The young father Joe was now destressed and the old mother silently started praying.

The story Birth is an excerpt from The Citadel. It relates how a medical fresher handles a child delivery case in a mysterious way and brings back life in a seemingly dead born child. In doing so he applies his medical text book knowledge as well as intuition. He finds that text book knowledge alone is not sufficient to handle such cases. The story expresses the anxiety and excitement of the doctor, Andrew Manson and his team.
In the beginning of the story, it is narrated how Andrew Manson who had recently passed out from a medical school, was returning home after a disappointing evening with his girl friend, Christine. It was midnight and when he reached near his house he was requested by Joe Morgan to attend to his wife immediately. She was expecting her first child. Andrew agreed to go despite late night.
At the patient's house, Andrew saw that the lady was being attended to by a midwife. Mrs.Morgan was very serious. Andrew examined the patient and could understand that it would take some time for the delivery to take place. He took a break. In this period Andrew's mind started wavering in stray thoughts. He started thinking about Branwell who was foolishly devoted to a woman who deceived him meanly. Then he thought of Edward page who was married to the ill-natured Blodwen, and was now living unhappily, apart from his wife. Andrew thought all marriages were a dismal failure. In the meantime Andrew had to go back to attend to the patient.
The subsequent section narrates how after an hour long harsh struggle a child was born towards the dawn. But the child was still born. When Andrew gazed at the lifeless form, he shivered with  horror. Then he looked at the mother. She required immediate attention. Andrew was to decide to whom to attend: the child or the mother.
Andrew gave the child to the nurse and turned his attention to the mother who was unconscious. Gradually, her heart strengthened and Andrew could now leave her to attend to the baby.
Andrew asked about the child. The midwife was thoroughly frightened. Taking it as a still child, she had dumped it beneath the bed amongst soaked newspaper. Andrew pulled out the child. The child was perfectly formed. Andrew could understand that it was a case of suffocation due to lack of oxygen and excess of carbon-dioxide in the blood. Andrew started giving the necessary treatment. He did not lose heart. The midwife again and again told him that it was a still born child. But Andrew did not heed. He continued with his treatment.
Towards the end of the story, a miracle happened he rubbed the child with a towel crushed and relaxed the little chest with both his hands. Thus, he tried to infuse breath into that limp body. The little chest gave a short, convoluted heave. The child started turning around. It was no longer still born.
At last Andrew heaved a sigh of relief. He handed the child to the nurse. He told the entire story to Joe standing outside and walked down the street with some miners with a sense of deep satisfaction that he could achieve something great.

  • Dr. Andrew Manson was returning home after a disappointing visit with his lady love, Christine and he had no notion that this night was going to mark a turning point in his life and career.
  • Exhausted, he found Joe Morgan, a poor miner, anxious and somewhat scared, waiting for him. Morgan’s wife was in labor, before the expected date. This is their first child in a marriage of nearly twenty years.
  • Joe Morgan seems to trust Andrew implicitly though many people were averse to modern medicine during those days. The presence of the midwife who had arrived before the doctor and Mrs. Morgan’s reluctance to take anesthesia reflect the lack of complete faith in modern medicine.
  • He reassures Mrs. Morgan’s mother telling her both the mother and the baby shall be fine.
  • Andrew waits in the kitchen reflecting on the information he gleaned earlier in the night which showed him that most marriages were dismal failures. However, he feels Christine is an exception.
  • The labor leads to a stillborn baby. The baby was born, but no sign of life, no breathing, nothing.
  • Initially, Andrew feels suddenly overwhelmed with the enormity of the situation and the many hopes he had dashed despite his initial reassurance.
  • In moments though, Andrew’s training kicks in. He instinctively decides to save the mother first, handing the baby over to the midwife.
  • The midwife, who has had no medical training, sees the body in her arms as a lifeless lump and places it under the bed among sodden newspapers.
  • Once the mother was out of danger, Andrew draws the baby out and quickly diagnoses the most probable cause for the still birth i.e. Asphyxia, pallida.
  • He recalls a method he had once observed through which a child had been successfully resuscitated.
  • He tried the simultaneous hot and cold dips to shock the body and get the heart to jump start, then rubbed the baby’s body with a rough towel crushing and releasing the little chest as a form of CPR.
  • On the verge of giving up, a medical miracle occurs. The child finally breathes. Notice the author’s description of the mucus as a joyful iridescent bubble.
  • Andrew redoubles his efforts till the baby is breathing freely and safe.
  • As he left the house, he realized that he had truly saved a life that night fulfilling the purpose of his profession. For the first time in his life, he felt he had done something ‘real’, something worthwhile.

Towards the end of the story, a miracle happened he rubbed the child with a towel crushed and relaxed the little chest with both his hands. Thus, he tried to infuse breath into that limp body. The little chest gave a short, convulsive heave and then another.Andrew redoubled his efforts The child was gasping and then came a cry. It was no longer still born.Andrew heaved a sigh of relief and uttered these words''I've done something real;oh God! I've done something real at last" At last Andrew heaved a sigh of relief. He handed the child to the nurse. He told the entire story to Joe standing outside and walked down the street with some miners with a sense of deep satisfaction that he could achieve something great.

?The story Birth is an excerpt from The Citadel.

When Andrew gazed at the lifeless form, he shivered with? horror.
Regular growth of population one of the major causes of poverty because:

a.?Higher population implies a greater dependency . The rapid growth of population, particularly among the poor, is considered one of the major causes behind Indian poverty. Poor people are illiterate and have traditional outlook. Hence, they are either ignorant of birth control measures or not convinced of the need of birth control.

b. They become the worst victims, the increase in number of ?family members become more of a burden / liability for them because their income is less to manage basic necessities of life and to look after large family.

c.? In addition to this, the bias in favour of the male child leads them to increase the family size irrespective of the consequences.

d.?Higher population implies lower per capita income. Also, it acts as a hurdle in the way of quality education, health care facilities and other facilities necessary for a quality life.

e.? Thus, high population leads to a low standard of living and aggravates poverty ?

The lesson begins with Joe walking towards his home after having an unpleasant time with his girlfriend Christine. Upon reaching home at around midnight, he finds Joe Morgan outside his house waiting for him. Joe and Susan had been expecting a child. Upon reaching, Joe tells Andrew that he is too anxious to be going inside, so Andrew walks in alone. He realises that there is still some time left. Susan's mother, who is wise but tense at the moment, offers him some tea. While he is sipping his tea, he begins thinking about his girlfriend. He recalls how his friends too are having a hard time in their relationships. On the other hand, he feels that marriage is meant to be something peaceful and cheerful. Thus, he finds himself in a conflict. Susan's mother tells him how worried Susan is worried for the child because the situation is complicated. After performing the procedure, the child is born, but us lifeless. Confused between addressing the child or the mom, who herself laid unconscious at the moment, he chooses to look after Susan first. He injected her with a few medicines and when he realised that her heart was regaining its strength, he turned towards the child. It was a boy in perfect shape. He suddenly remembered that it was a case of restricted oxygen supply and thus, he instructed the nurse to get hot and cold water. The nurse, reluctant at first, got him what he needed and he juggled the child in hot and cold water simultaneously. After all his efforts, the child still lay lifeless. All others are already lost hope, while he continued with the procedures. Finally, the child heaved and got his breath. His body was no longer still, leaving everyone surprised. The nurse muttered words of prayer while holding the child. The grandmother was also praying, while Susan still laid unconscious. Exhausted, Andrew left while informing Joe that all is well. While he was walking home at around five in the morning, his heart was full at having accomplished something real in his life.
