Far point of eye means the farthest point/object that can be seen by the eye. 

Now, if visible light of visible intensity fall on our eye we can see very very far off objects e.g. stars in the sky. That is why the farthest point for normal human eye is infinity. When eye is made to focus at the far point, the eye lens has the lowest power.

Near point of eye refers to the shortest distance that can be seen clearly by human eye. 

It is 25 cm for a normal eye, when eye is made to focus at the near point, the eye lens has the highest power.

the minimum distance between the object and the eye(lens)so that a clear image is formed on the retina is called the least distance for clear vision or near point of the eye.the near point of a normal eye is 25cm.
