Is the indentification of gender clarified at birth? If not, then is it a transgender? Or does a trangender person has its gender clarified at birth?

Identification of gender is clarified at birth. 

Identification of gender in case of trans people is also clarified during birth, but with age, these people start feeling their identity differs from the sex they were assigned at birth.

Transgender is the term use to describe people whose gender identity or gender expression differs from biologically assign sex. 
Normal male or female posses 46 chromosome. Transgender condition is formed due to sex chromosomal abnormality. The abnormality occurs due to non disjunction (is the failure of the chromosome pair to get separated during the meiosis type of cell division) of chromosome during cell division. Thus these people have increased chromosomal ratio than the normal people.

Transgender person has a gender identity that does not match the sex they were assigned at birth. So, a child who was assigned male on their birth certificate and who identifies as a girl is transgender (sometimes this term is shortened to “trans”).
