The report has some mistakes-

Please use the same spelling for Calcutta.
Try to use smaller sentences.
The sentence " As the driver failed to stop the bus he ...... driver and his friend out to death" has grammatical mistakes.
Try rewriting it.

The time of the accident has to be given in the initial paragraph of the report.
