The main difference between strong acids (or bases) and weak acids (or bases) is the degree of dissociation. 
Strong acids (or bases) dissociates completely, releasing all the hydrogen ions (or hydroxyl ions).
Weak acids (or bases), dissociates partially.
So, a strong acid is the one which dissociates completely to provide hydrogen ions, while a weak base is the one which dissociates partially to provide hydroxyl ion.
e.g., HCl is a strong acid while NH4OH is a weak base.
